The four VLTP forms at a glance

All four PDFs are fillable and hosted on opm.gov. Some agencies use their own equivalents inside HR systems — an agency form asking for the same information satisfies the regulation.

Which OPM form do you need?

• You need donated leave → OPM Form 630, submitted to your own agency. Our recipient guide walks through every field and the 10-day decision that follows.
• You're donating to a colleague in your agency → OPM Form 630-A. The regulation draws agency boundaries in ways that aren't always intuitive — the military departments, for example, each count as a separate agency. When in doubt, your HR office knows the boundary.
• You're donating to an employee of another agency → OPM Form 630-B. Cross-agency transfers also need the recipient's agency to accept them — automatic for family members, discretionary otherwise, as explained in the donor guide.
• You work in HR and a recipient is transferring agencies → OPM Form 630-C documents the donated-leave balance moving with them.

When someone pledges hours on LeaveTransfer.org, we apply exactly this logic automatically: pledges from the recipient's own agency are pointed to Form 630-A, and pledges from any other agency to Form 630-B — so donors always leave with the right paperwork.

What the donor forms make you certify

Forms 630-A and 630-B are one page each, but the certifications matter. You confirm the recipient is not your immediate supervisor, that the donated amount is within your annual cap ("not more than half the hours I will earn this year"), and — the one that surprises people — that the donation is not revocable. Unused hours come back only through pro-rata restoration when the emergency ends, never on request.

Where do the forms go?

Every form goes to your own agency's HR or timekeeping office. OPM publishes the forms but processes none of them, and LeaveTransfer.org never touches them — pledges made here become real transfers only when the donor files the form through their agency. Many agencies accept the fillable PDF by email or through their HR portal; ask your HR office which channel it prefers.

Different program, different forms: the ELTP series

If you're donating after a major disaster through the Emergency Leave Transfer Program, the 630 series doesn't apply — ELTP uses OPM Forms 1637 (recipient application), 1638 (donation), and 1639 (transfer of donated leave). Agency leave banks, likewise, use their own enrollment and withdrawal forms set by each leave bank board rather than the 630 series.

Frequently asked questions

What is the difference between OPM Form 630-A and 630-B?

Both request a donation of annual leave to a named leave recipient. Form 630-A is used when the donor and recipient work for the same federal agency; Form 630-B is used when they work for different agencies. Cross-agency donations on 630-B also require the recipient's agency to accept the transfer.

Where do I send my completed OPM Form 630 or 630-A/630-B?

To your own agency's HR or timekeeping office. OPM publishes the forms but does not process applications or donations, and no third-party website can process them either. Your agency verifies your leave balance and executes the transfer.

Can I submit VLTP forms electronically?

The OPM PDFs are fillable, and submission channels vary by agency — many accept the forms by email or through internal HR systems. Confirm with your HR office which channel it prefers.

Is there a form to cancel or take back a leave donation?

No. Forms 630-A and 630-B include an explicit certification that the donation is not revocable. The only way donated hours return to you is automatic pro-rata restoration if the recipient's medical emergency ends with leave unused.

Do I use the 630 forms for an agency leave bank?

No. The 630 series is specific to the Voluntary Leave Transfer Program. Leave banks are governed by 5 CFR Part 630, Subpart J, and each agency's leave bank board sets its own membership and withdrawal forms.
